TCA CROSS for Icepick Scars
Ice pick scars are narrow, sharp pits that extend deep into the skin, and they are among the hardest acne scars to change. Because broad resurfacing mostly smooths the surface, doctors often turn to a focused method. TCA CROSS applies strong acid only inside each pit, which is the main thing that sets it apart.
How it works
CROSS stands for chemical reconstruction of skin scars. A high-concentration trichloroacetic acid (TCA) solution is placed into each scar with a fine applicator, such as a toothpick or small stick, with care to keep the acid within the pit. The acid causes a small, controlled injury at the base, the area frosts white, and the body then rebuilds collagen to raise the pit gradually. Because the acid stays inside each scar, the surrounding skin is spared. Improvement is gradual and partial, and varies by person.
Who it may suit
- Narrow, deep ice pick scars
- Small boxcar scars with steep edges
- People who want a quick, low-intensity procedure
- Combining with laser or microneedle RF for other scar types
The treatment day
- Map the scars. The doctor identifies which pits suit TCA and notes areas that need other methods.
- Clean and prepare. The skin is degreased, and the surrounding area is protected from stray acid.
- Apply acid into each scar. The solution is placed in each pit until a white frost appears, then stops.
- Rest and plan repeats. Treated dots crust over, and the doctor suggests how long to wait before repeating.
Aftercare
- Small dark crusts form and fall off over about a week; do not pick them.
- Apply any ointment the doctor recommends and keep the area clean.
- Use sunscreen carefully to reduce the chance of dark marks.
- Report spreading redness, pus, blisters or persistent pain.
Things to know first
- Brief stinging is common, and treated dots look dark and crusted for days.
- Possible side effects include post-inflammatory darkening, lingering redness, widened scars, infection and, rarely, new scarring.
- Acid that spreads beyond the pit can injure normal skin, so technique matters.
- Darker skin tones, recent isotretinoin use, keloid tendency, active infection and pregnancy call for a doctor's review.
- It is not intended for rolling scars, large areas or flat pigment.
How TCA CROSS compares with nearby options
The central comparison is with broad resurfacing. A fractional laser spreads energy across the whole area, which suits texture and shallow scars but has limited reach into a narrow, deep pit. TCA CROSS puts the stimulus right at the base of the pit and leaves the rest of the skin alone, so the recovery is localized to tiny dots rather than the full face.
Subcision targets a different shape: rolling scars held by tissue bands. A face with both rolling and ice pick scars may need both methods, each used on its matching scar. Microneedle RF and microneedling can follow to refine texture, and a punch technique is a surgical alternative the doctor may mention for the deepest pits.
TCA CROSS does not tighten, treat pigment or soften broad shallow depressions well. If your marks are flat or wide, other categories fit better. A doctor can sort out your scar types in person.
TCA CROSS for Icepick Scars: common questions
Why is TCA used only inside the scar?
Concentrated acid can injure healthy skin, so it is placed carefully within each pit to trigger a local repair without wide damage.
How many sessions are common?
Often several, spaced a month or more apart, because each pit improves a little at a time.
Does it work for rolling scars?
Not usually. Rolling scars are tethered and respond better to subcision or other methods.
Can it darken my skin?
Post-inflammatory darkening can occur, especially in darker skin tones. Sun protection and careful technique help limit this.
