Erbium Fractional Laser

Some people want more than gentle laser or needling can offer, but not the week-long crusting that CO2 resurfacing can bring. Erbium fractional lasers occupy that middle ground, and the comparison with CO2 and non-ablative options is what makes the choice clearer.

How it works

An Er:YAG laser (2940 nm) is absorbed very strongly by water, so it removes thin layers of tissue with little heat spreading to the surrounding skin. In fractional mode it creates microscopic columns of removed tissue, and the skin heals by producing new collagen and a smoother surface. Because less heat reaches deeper layers, there is less coagulation and tightening than with CO2, but the redness and swelling are often shorter. Some systems combine erbium with deeper heating modes. The depth is set by the doctor, and outcomes vary by person.

Who it may suit

  • Shallow to moderate acne scars and surface texture
  • Fine lines and early sun damage
  • Rough texture with slightly enlarged-looking pores
  • Those wanting resurfacing with a shorter, more predictable recovery than CO2

The treatment day

  1. Choose the depth. The doctor selects a depth and density that fit the scars, skin tone and how much recovery you can accept.
  2. Numb the skin. Numbing cream is applied beforehand, and the face is cleaned.
  3. Laser passes. The fractional beam is moved over the area, usually with a sharp snapping feeling.
  4. Soothe and instruct. A calming ointment is applied and specific aftercare is explained.

Aftercare

  • Expect redness, mild swelling and fine crusting or flaking for a few days up to about a week.
  • Apply the recommended ointment or moisturizer and avoid picking or scrubbing.
  • Use gentle cleansers and daily sunscreen; avoid direct sun until the skin has healed.
  • Contact the doctor for increasing pain, discharge, blisters or fever.

Things to know first

  • Possible side effects include redness, swelling, oozing, dryness and temporary darkening of the skin.
  • Infection, cold sore reactivation and, uncommonly, scarring or lasting pigment changes can occur.
  • Darker skin tones have a higher chance of discoloration and need cautious settings.
  • Recent isotretinoin use, active infection, keloid tendency, pregnancy and fresh tanning call for a doctor's review.
  • It may be less effective than CO2 for deeper scars or lax skin.

How erbium fractional compares with nearby options

Think of the three fractional lasers as a ladder. CO2 is the strongest and warmest, erbium is in the middle with a cleaner, shallower ablation, and non-ablative 1550 nm lasers are the gentlest because they leave the surface intact. Erbium usually trades some tightening for a faster return to a normal-looking face. If you want the most change and accept a longer recovery, CO2 is the closer comparison.

Against microneedle RF, erbium works on the surface and upper dermis, so it tends to be the more direct choice for visible roughness and fine lines, while microneedle RF heats deeper with less surface injury and is often discussed for pores and lax skin. For deep, tethered rolling scars, subcision is used alongside either, and for narrow ice pick scars, TCA CROSS is a targeted option.

Erbium does not treat pigment disorders such as melasma well and does not lift sagging. A doctor can compare these categories against the scar or texture you have.

Erbium Fractional Laser: common questions

How is erbium different from CO2 fractional?

Erbium removes tissue with less heat spread, so recovery is often shorter, but tightening is usually less than with CO2. Depth settings can narrow the gap.

Is there downtime?

Usually a few days to about a week of redness, swelling and fine crusting. It depends on the depth chosen.

Does it work for deep acne scars?

It may soften shallow to moderate scars. Deep or tethered scars often need subcision or other techniques as well.

How many sessions are common?

Often a short series spaced several weeks apart, adjusted to how your skin responds.
